Service Connector Program

The Service Connector program seeks to support Dehcho Members with knowledge of, and access to, health and wellness-related resources and services available to them.

This program was created by the Dehcho First Nations Health & Wellness Division as a response to challenges that Dehcho Members are currently experiencing when trying to access important health & wellness resources and services. This program also seeks to support Dehcho Members with access to resources and services in a safe, supportive, Dene way, that supports Members leading their own health and wellness journey.

Please note that we do not provide direct services, recommendations, or emergency services, but instead support Dehcho Members with access to the health and wellness options available to them.

How Connection Works

  1. Reach out to a connector. Dehcho Members have priority in this service.
  2. Your connector will listen to learn more about your health and wellness needs and wants. Your connector may need to do some extra research in order to connect you with some options. Once ready, your connector will provide you with information about options related to services and resources available. The connectors do not provide services themselves, or any recommendations, they are there to support your health and wellness journey.
  3. Once options are provided to you, Dehcho Members may choose to register fully with the service, which means that your dedicated connector stays in touch in order to walk alongside you in your journey. It’s completely up to you. Your connector will stay in touch to see if the option provided is right for you. If needed, your connector will chat more about alternative options.
  4. The end of your journey is up to you! You decide when you have reached your health and wellness goals, or if you have questions about options for another health and wellness goal.
